WELL WHAT IS THAT ?
In the attempt to make fat removal more
effective, methods and instruments have evolved that have
increased our abilities to remove fat with less bleeding
and with a better chance of a pleasing contour. The instrumentation
has also increased the amount of fat that can be removed
and the smoothness of the skin after removal.
One of the most important aspects of liposuction
surgery is to have skin that was smoother than
it was before the operation. This is not possible
in all cases. But this is our goal.
The most recent development is subsonic
liposuction. In my experience it can allow for the more
complete removal of fat and a better chance for a tighter,
more pleasing skin contour. I use this method whenever possible.
Subsonic liposuction involves the use of
a rapidly vibrating suction head. The cannula used is quite
similar to the ones that we use during traditional liposuction,
however the cannula is hooked to a vibrating machine. The
machine causes the head of the cannula to move back and
forth approximately one-eighth to one-quarter of an inch.
It moves at speeds of three to four thousand times a minute.
This differs from ultrasonic liposuction in that ultrasonic
cannulas vibrate at 12 million times a second and the tip
of the cannula moves only a few thousands of an inch.
The subsonic liposuction machine in my
experience has been one of the more beneficial machines.
It is easy to use and in my hands. There are somewhat less
of a risk in using sub-sonic machine than when using ultra-sonic
equipment. There certainly is a place for both techniques.
The sub-sonic technique is one that will be use in most
every case.
One of the benefits that I have noticed
with this machine is the fact that there is less bleeding.
There is also less tingling and discomfort than when the
ultrasonic machine is used. The question arises, is there
a place for ultrasonic liposuction? Yes. It has its best
use in very tough tissues such as the hip area and also
in areas of fairly loose, thin skin - such as the medial
thigh area. Ultrasonic liposuction in theory can tighten
the skin and we have seen this. This seems to occur but
to a lesser degree with subsonic liposuction.
